Internal patient ID

Help

This field can be used to enter an internal ID or reference number. It is available for internal use only and is not transferred to Healthdata. If there is an integration between HD4DP and your organisation's patient administration system, additional patient identification information such as date of birth, sex and SSIN can be automatically enriched based on this internal patient ID. Please do not fill in the social security identification number (SSIN) in this field. The SSIN must be encoded in the patient ID field.

Generated

Help

The patient_id|generated column can be set to 'TRUE' for an automatically generated patient_id.This column should only be added in case the patient_id is empty. If the column is set to 'TRUE', a patient_id is automatically generated based on the name, first name, date of birth and sex of the patient.

Quality of the specimen

Help

This variable is used for cervical smears (cytology) only, not for histological samples. Accepted codes : ● SUF=Sufficient: the sample is of sufficient quality to result in a reliable diagnosis. ● INSU= Insufficient: no reliable test result could be determined or the specimen could not be evaluated. The code ‘INSU’ should only be used if no reliable diagnosis can be established. This implies that a new smear should be taken.

HPV high risk types detected

Help

Following 13 HPV-types are considered by the BCR as high risk HPV-types: 16,18,31,33,35,39,45,51,52,56,58,59,68 and should be coded if your HPV test can discriminate different genotypes. Multiples HPV can be entered and must be separated by commas ','. Possible codes (meaning of the codes): ● HP16 (HPV Type 16) ● HP18 (HPV Type 18) ● HP39 (HPV Type 39) ● HPOT (HPV high risk genotype different from HPV16/HPV18, if no genotyping other than HPV16/HPV18 is possible)
