1 Identification

Section help

Dit formulier is enkel bestemd voor artsen die deelnemen aan de huisartsenpeilpraktijken, een register bedoeld voor wetenschappelijk onderzoek.
Indien u interesse heeft om deel te nemen aan deze registratie, graag eerst contact opnemen met Viviane.VanCasteren@wiv-isp.be vooraleer te registreren.

Problematisch middelengebruik
Registreer bij het EERSTE contact elke patient die bij u zelf een NIEUWE behandeling start voor zijn of haar middelengebruik.
Registreer alleen UW EIGEN patiënten, geen patiënten die u voor de eerste keer ziet, bijvoorbeeld tijdens een wacht. De behandeling bij de huisarts START bij het eerste face-to-face contact over dit probleem.
De behandeling is GESTOPT wanneer er al 6 maanden geen contact meer geweest is tussen de patiënt en de huisarts m.b.t. het problematisch middelengebruik.
