Suicide en suicidepogingen
Registreer ALLE suïcidale handelingen waarmee u in uw praktijk te maken had, dit wil zeggen van UW patiënten + van de patiënten die u tijdens een WACHTDIENST zag. Registreer dus ook de gebeurtenis als u niet de eerste hulpverlener was of pas achteraf op de hoogte bent gebracht.
Tenzij de patient overleden is worden na 4 weken de 1e follow-up gegevens gevraagd en na 6 maanden de 2e follow-up gegevens.
Suïcidale handelingen zijn alle handelingen van opzettelijke zelfverwonding of zelfintoxicatie met fatale afloop (suicide) of niet fatale afloop (suïcidepoging).
Een suïcidepoging is een handeling, met een niet fatale afloop, waarbij een individu opzettelijk, weloverwogen een ongewoon gedrag stelt of initieert, dat zonder interventie van anderen zal leiden tot zelfverwonding (of zelfvernietiging), of waarbij een individu opzettelijk een stof inneemt in een grotere dan de voorgeschreven of algemeen aanvaardbare therapeutische dosis, met de bedoeling verlangde verandering(en) te bewerkstelligen.
